چکیده

Background: APOE genotype provides information on risk of developing Alzheimer’s disease (AD), but disclosure is discouraged. We examined the impact of APOE disclosure in a prospective randomized controlled trial: the Risk Evaluation and Education for Alzheimer’s Disease (REVEAL) Study. Methods: We randomly assigned 162 asymptomatic adult children of AD patients to Genotype Disclosure (GD) or Genotype Non-Disclosure (GND) groups. We measured anxiety, depression symptoms and test-related distress 6 weeks, 6 months and 1 year after disclosure. Results: Changes in time-averaged measures of anxiety (p = 0.84), depression symptoms (p = 0.98) and test-related distress (p = 0.61) did not differ between GD and GND groups. Secondary comparisons between those with higher risk (GDε4+) and the GND group revealed no significant differences. The GDε4group showed a significantly lower level of test-related distress in comparison with the GDε4+ group (p = 0.01). Persons with clinically meaningful changes in psychological outcomes were distributed evenly among the GND, GDε4+ and GDε4groups. Baseline scores of anxiety and depression symptoms were strongly associated with postdisclosure scores of these measures (p < 0.0001 for each). Conclusion: Disclosure of APOE genotype to adult children of persons with AD did not result in significant short-term psychological risks to those who received genotype information or to those who learned they were APOE ε4+; distress was reduced among those who learned they were APOE ε4-. Persons with high levels of emotional distress before undergoing genetic testing were more likely to have emotional difficulties after disclosure. (ClinicalTrials.gov number NCT00571025.)

Health behavior changes after genetic risk assessment for Alzheimer disease: The REVEAL Study.

Risk information for Alzheimer disease (AD) may be communicated through susceptibility gene disclosure, even though this is not currently in clinical use. The REVEAL Study is the first randomized clinical trial of risk assessment for AD with apolipoprotein E (APOE) genotype and numerical risk estimate disclosure. We examined whether APOE genotype and numerical risk disclosure to asymptomatic in...

بررسی پلی مورفیسم ژن APOE در بیماران ایرانی مبتلا به آلزایمر

Background: Alzheimer's disease is a multi-factorial disorder. The genes involved in Alzheimer's disease act as risk factors. The aim of this study was to investigate the APOE gene polymorphism in patients with Alzheimer's disease in Iran. Materials and Methods: In this analytical-descriptive study, 50 patients with Alzheimer's were investigated. Polymorphism of the APOE gene was investigated ...

Association study between DNA methylation and genetic variation of APOE gene with the risk of coronary artery disease

Coronary artery disease (CAD) is a common health problem with a high rate of disability and death. Dyslipidemia and altered metabolism of Apo-lipoproteins are involved in the CAD pathogenesis. The current study investigated two common polymorphisms (rs429358 and rs7412) and promoter DNA methylation status of APOE in the Iranian CAD patients and control subjects. Two hundred angiographi...

Identification of Genetic Polymorphism Interactions in Sporadic Alzheimer’s Disease Using Logic Regression

Objectives: Genetic polymorphism interactions are among the important factors in affliction with complex diseases like Alzheimer’s disease. The important goal of genetic association studies is to identify a combination of polymorphisms and measure their importance in increasing the risk of occurrence of such diseases. In this study, feature selection approach of logic regression was used to ide...

No eVect of the á1-antichymotrypsin A allele in Alzheimer’s disease

The apolipoprotein E (ApoE)-å4 allele is associated in a dose dependent manner to an increased risk for Alzheimer’s disease. However, the ApoE-å4 allele eVect does not account for all patients with Alzheimer’s disease, and the existence of other genetic risk factors has been postulated. Kamboh et al reported an association between Alzheimer’s disease and the A allele of á1-antichymotrypsin (Aac...
